State places psychologist James Polk on probation

On April 22, 2013, the Virginia Board of Psychology placed psychologist James P. Polk on indefinite probation for failure to document a patient’s informed consent for services provided; performance of a neuropsych evaluation without a referral from the patient’s physician or administrator and use of an out-of-date testing instrument, among other things.
