State denies application of social worker Jeffery Taylor

On August 1, 2011, the Arizona Board of Behavioral Health Examiners (AZBBHE) denied the licensed clinical social worker application of Jeffery Taylor due to unprofessional conduct and failure to meet minimum licensure standards.
The AZBBHE’s report states that Taylor failed to improve his documentation following a February 2007 performance evaluation. A subsequent evaluation in April 2008 rated him below expectation and he was put on a 30-day corrective plan—a disciplinary action. He was terminated shortly after.
Following his termination, Taylor made repeated and ongoing attempts to contact the Vice President of his former place of employment, via lengthy e-mails and voice messages.
He filed a 38-page complaint with the AZBBHE against the Vice President. On his application for a licensed clinical social worker license, he made several misrepresentations about his former employment.
