State declines to issue license to counselor applicant Tamara Van Berkel

On April 2, 2012, the Arizona Board of Behavioral Health Examiners denied the licensed associate counselor (LAC) application of applicant Tamara Van Berkel for unprofessional conduct. According to the Board’s report, van Berkel misrepresented on her LAC application that she was employed at a university when in fact she had been terminated some months earlier for failure to meet expectations. She also failed to disclose three psychiatric hospitalizations. Further, she failed to respond to Board inquiries. Additionally, court-ordered urine analyses indicated positive results in February 2009 and a series of dilute samples. “Applicant demonstrated a pattern of non-disclosure and/or inaccurate representations,” according to the Board’s report.
