State Issues Accusation Against CA Psychiatrist Soleyman Mirakhor Over Excessive Prescribing

On September 13, 2018 the Medical Board of California issued an Accusation against psychiatrist Soleyman Mirakhor.
The Board’s 37-page document lists out charges against Dr. Mirakhor which include Gross Negligence, Repeated Negligent Acts, Incompetence, Excessive Prescribing, Prescribing without Medical Indication, Inadequate and Inaccurate Record Keeping, and Unprofessional Conduct, all relative to four patients.
One of the patients was hospitalized with respiratory failure and nearly died from drug-to-drug interactions, due not only to Dr. Mirakhor’s excessive prescribing but also his failure to run a Controlled Substance Utilization Review & Evaluation System (CURES) report--a standard action from which he would have discovered that the patient was receiving prescriptions from another practitioner which ahd known adverse interactions with the drugs he was prescribing the patient.
