On November 22, 2013, licensed clinical social worker Daily Miller voluntarily relinquished her license to the Florida Board of Clinical Social Work, Marriage & Family Therapy and Mental Health Counseling. Miller was convicted June 28, 2013 of one count conspiracy to commit health care fraud and wire fraud; five counts of wire fraud; two counts of health care fraud; and one count of conspiracy to defraud the United States and to pay and receive kickbacks in connection with a federal health care benefit program.

On November 22, 2013, licensed clinical social worker Martin Ludwig voluntarily relinquished his license to the Florida Board of Clinical Social Work, Marriage & Family Therapy and Mental Health Counseling. The Florida Department of Health suspended Ludwig’s license in November 2012 for, among other things, failing to meet the minimum standards of performance in professional activities and undertaking activities for which he was not qualified.

On November 22, 2013, the Florida Board of Clinical Social Work, Marriage & Family Therapy and Mental Health Counseling revoked the license of clinical social worker Monel Herard. The Board’s document states that Herard committed sexual misconduct with two clients, both female, by requesting photographs of their breasts, asking them about their sexual activity and, in one case, warned the client to cooperate with his requests because it was he who was conducting the evaluations for the client to complete her probation.

On November 21, 2013, the Ohio Counselor, Social Worker and Marriage & Family Therapist Board reprimanded social worker Kaitlyn Pecherkiewicz for inaccurately documenting client records.

On November 4, 2013 the Minnesota Board of Social Work reprimanded social worker Tyra L. Taylor for practicing social work without a license.

On November 1, 2013, the Utah Division of Occupational and Professional Licensing issued a license to certified social worker Gary Merrill Poulsen but placed the license on probation for five years with conditions and restrictions. Poulsen engaged in unprofessional and unlawful conduct prior to submitting his application for licensure.

On November 1, 2013, clinical social worker Scott A. Peterson voluntarily surrendered his license to the Utah Division of Occupational and Professional Licensing. Peterson engaged in an inappropriate sexual relationship with an adolescent client.
