The Board initially placed Jacobson on indefinite probation on June 14, 2011.

The message was personal and expressed a desire to “know each other better.”

The patient told him not to contact her again under any circumstances but he did, the same day.

The order additionally states that he billed clients for in-person therapy that were actually phone calls and billed another client for a session which she did not attend.

Jacobson was placed on practice supervision but ultimately informed the Board that he would not be renewing his license because his professional liability insure had refused to renew his insurance following the Board’s disciplinary action.
