On January 17, 2014, the Ohio Counselor, Social Worker and Marriage & Family Therapist Board (“Board”) revoked the license of social worker Brian M. Davis for failure to comply with the Board’s continuing education audit requirement.

On January 17, 2014, the Board revoked the license of social worker Jane A. Cox for failure to comply with the Board’s audit requirement after renewing her license through May 2015.

On January 17, 2014, The Board revoked the license of social worker Diana Merriweather for failure to comply with the Board’s continuing education audit requirement.

On January 9, 2014, social worker Craig R. Campbell surrendered his license to the Board. In August 2013, the Board received a complaint against Campbell, alleging possible impairment. The Board ordered him to obtain an impairment evaluation. Campbell surrendered his license in lieu of disciplinary action.
