Nebraska disciplinary actions against mental health practitioners, 9/12 to 7/13

The Nebraska Dept. of Health and Human Services took the following disciplinary actions:

On July 3, 2013, the state accepted the voluntary surrender of licensed mental health practitioner Katherine Zupancic’s license. According to the state’s documents, Zupancic was found in September 2012 to have violated the False Medicaid Claims Act. She was convicted and ordered to pay $299,355.

On June 21, 2013, the state placed counselor Carol Ann Lanham Frost on probation for three years. The states documents state that Frost engaged in dual relationship with a client and engaged in departures from the standard of care.

On June 5, 2013, the state censured  mental health counselor Carole Jorden and added two years to the end of the current three-year probation license she is now working under. The state’s documents indicate that, among other things, in 2007, Jorden loaned money to a patient and held the patient’s diamond wedding ring as collateral and that in 2011, she was disciplined for submitting numerous claims to Medicaid that was not reimbursable due to lack in specific information. She was additionally excluded from state Medicaid.

On May 8, 2013, the state issued James D. Sewell a probationary license to practice as a provisional mental health practitioner, with terms and conditions including submission to random body fluid screens. The states’ document shows that Sewell has several alcohol-related criminals convictions from 2005 to 2011.

 On September 20, 2012, the state placed provisional mental health practitioner Aaron Engelbart on probation for vive years. The state’s documents show that Engelbart has several convictions for driving under the influence.
