On October 24, 2013, the Michigan Department of Licensing and Regulatory Affairs Bureau of Health Care Services (Department) reprimanded licensed Masters social worker Debra Ruth Fairchild and imposed upon her a fine for betrayal of professional confidence.

On October 24, 2013, the Department suspended licensed Bachelor social worker Sondra Michelle Malone on charges of failure to meet continuing education requirements.

On September 24, 2013, the Department placed licensed Masters social worker Shawn Leigh Gauthier on probation and imposed a fine for violation of general duty/negligence.

On September 24, 2013, licensed Bachelor social worker Sally Shear Hardy surrendered her license to the Department on charges of failure to meet continuing education requirements.

On September 24, 2013, licensed Bachelor social worker Jefferson Nichols, Jr. surrendered his license to the Department on charges of failure to meet continuing education requirements.

On September 24, 2013, the Department placed licensed Masters social worker Rebecca Ratliff on probation and limited/restricted his license for negligence and incompetence.

On September 24, 2013, the Department placed the license of licensed Bachelor social worker Lisa D. West on probation and imposed a fine for failure to meet continuing education requirements.

On September 11, 2013, the Department placed social worker Alice M. Raby on probation and imposed a find for failure to comply with continuing education requirements.

On September 1, 2013, the Department summarily suspended the license of Masters social worker Jo Ann McGhee for unethical business practice, failure to report a criminal conviction, unprofessional conduct and lack of good moral character.

On August 30, 2013, the Department placed Bachelor social worker Dana Leigh Vance on probation and imposed upon her a fine, for failure to meet continuing education requirements.
