Michigan social worker disciplinary actions, July-August 2013

On July 23, 2013, the Michigan Department of Licensing and Regulatory Affairs Bureau of Health Care Services (“Department”) placed licensed Bachelors social worker Timothy Michael Smith on probation and imposed a fine. The Department’s document states that he failed to meet continuing education requirements.
On July 23, 2013, the Department placed licensed Bachelors social worker Casandra R. Dysard on probation and imposed a fine for failure to meet continuing education requirements.
On July 28, 2013, the Department summarily suspended licensed Masters social worker Renee Lynne Schmidt for violation of general duty/negligence, mental/physical inability to practice and substance abuse.
On August 6, 2013, the Department placed licensed Bachelors social worker Maryann O’Connor Perillo on probation and imposed a fine for failure to meet continuing education requirements.
On August 6, 2013, the Department reprimanded licensed Bachelors social worker and licensed professional counselor Mary Marcia Young. They also imposed a fine and placed her on probation for failure to meet continuing education requirements.
On August 16, 2013, the Department placed licensed Bachelors social worker Paula Richards Smith on probation and imposed a fine for failure to meet licensing requirements.
On August 20, 2013, licensed Masters social worker Kathryn Elizabeth Mizzi voluntarily surrendered her license to the Department, which also imposed upon her a fine for lack of good moral character and violation of the state health code.
On August 22, 2013, the Department limited the license of licensed Masters social worker Judy Lynn Muncey and placed her on probation for lack of good moral character and violation of general duty/negligence.
On August 22, 2013, the Department imposed a fine on licensed Bachelors social worker Janet Irene Zielasko and placed her on probation for failure to meet continuing education requirements.
