On October 27, 2013, the Michigan Department of Licensing and Regulatory Affairs Bureau of Health Services (Department) placed licensed professional counselor Linda Nina Smith-Cummings on probation for a technical violation of the Michigan Public Health Code.

On October 13, 2013, the Department imposed a fine on licensed professional counselor Erin Meg Elsen-Bertke for violation of general duty/negligence and a technical violation of the Michigan Public Health Code.

On September 13, 2013, licensed professional counselor Joseph A Mitchell voluntarily surrendered his license to the Department on charges of negligence and incompetence.

On September 13, 2013, the Department suspended licensed professional counselor Paula Jo Paszke for negligence and incompetence.

On September 13, 2013, the Department imposed a find on licensed professional counselor Johnny Edward Cook and placed him on probation for negligence, incompetence and a technical violation of the Michigan Public Health Code.
