Medical Board Disciplines Psychiatrist David D. Gulden for Substance Abuse

On January 12, 2019, the Minnesota Board of Medical Practice suspended the license of Apple Valley psychiatrist David Gulden (14665 Galaxie Ave. Ste. 140, Apple Valley, MN 55124). The Board stayed the suspension, contingent on Dr. Gulden’s compliance with various terms and conditions.

According to the Board’s document, the reason for this disciplinary was that Dr. Gulden had relapsed into chemical abuse. Gulden entered treatment for chemical dependency and mental health issues in April 2017 and in May of that year, enrolled in the Health Professionals Services Program (HPSP), agreeing to abstain from alcohol, controlled substances and mood-altering substances, and refrain from practice until HPSP lifts the restriction, and other conditions including submitting to unannounced fluid drug screens.

Dr. Gulden tested positive on two occasions in March and April 2018, failed to report them to HPSP and, when contacted by HPSP, denied relapsing. Dr. Gulden entered residential chemical dependency treatment in both May and November 2018.

Dr. Gulden is required to participate in HPSP. Any violations of the program may result in suspension of his license. His license is placed on conditional status for no less than four years.

The Board suspended Gulden’s license in September 1992 based on chemical dependency and illness and place him on a conditional license for five years.
