Florida revokes counselor Jennifer Baxt's license for fraud, falsified records

The Florida Department of Health issued a Settlement Agreement suspending Baxt’s social work and counseling licenses for five years, among other disciplinary measures, including a reprimand and $2,000 fine. In July 2008, the state filed a complaint against Baxt, alleging that she had submitted bills and received payment for services she had actually not conducted with a particular minor patient. Additionally, the state alleged that Baxt had created a fraudulent monthly visit log for services to the client; falsified progress reports for the client and forging the client’s parent’s signature on the progress report sheets. She was subsequently terminated from her employment. It was later discovered that she had falsified visit logs, progress reports and forged the signatures of ten other clients and/or their caregivers, receiving more than $6,700 in Medicaid reimbursements for services she did not provide.
