On March 8, 2013, professional counselor Travis Waits permanently surrendered his license to the Oregon Board of Licensed Professional Counselors and Therapists (Oregon Board). The Board’s document cites Waits’ sexual involvement with a client, dual relationship, breach of confidentiality, exceeding the scope of competence.
