Counselor Herbert Warner surrenders license for misrepresentations

On June 6, 2013, professional counselor Herbert R. Warner surrendered his license to the Arizona Board of Behavioral Health Examiners.
The Board’s document states that Warner, on his application for licensure, represented that he had been in private practice in the state of Washington prior to applying for licensure in Arizona when, in fact, he had been employed at three behavioral health facilities in Washington and another in Arizona.
The document further details other misrepresentations that Warner made regarding his employment history as well having been charged with a misdemeanor for showing up at work with a firearm and failing to report the charge to the Board within ten days, as required, among other things.
