Counselor Erin Michalsky failed to provide one-on-one supervision to adolescent

On May 25, 2012, the Washington State Department of Health (DoH) suspended indefinitely the license of counselor Erin T. Michalsky. The DoH’s Findings of Fact states that on September 7, 2010, Michalsky, who was employed at an inpatient mental health facility, failed to provide one-on-one supervision to an adolescent client to whom she was assigned to provide such supervision. Michalsky failed to respond to the DoH’s correspondence about the complaint and a follow-up letter regarding her obligation to cooperate with the DoH’s investigation—neither of which she responded to.
