Arizona behavioral health authority denies application of social worker Sally Davis

On November 3, 2011, the Arizona Board of Behavioral Health Examiners (AZBBHE) denied the licensed clinical social worker license application of Sally Davis, due to unprofessional conduct.
Davis was employed from November 2003 to August 2009 at a behavioral health services agency but was terminated following a formal hearing regarding allegations of borrowing money from clients; disclosing personal problems to clients during therapy sessions; socializing with clients and failing to maintain appropriate professional boundaries with clients.
The AZBBHE’s report notes that Davis has “a history of behavioral…issues including plans to harm the client that accused her of borrowing money.” Davis also failed to inform her subsequent employer of her prior termination, as is required.
