Alcohol history, relapse results in state denying license to counselor

On November 7, 2013, the Arizona Board of Behavioral Health Examiners denied the application of associate counselor applicant John Jaksich for unprofessional conduct. The Board’s document specifically states that Jaksich disclosed a long history of alcohol abuse, treatment and relapse; listed his sobriety date as August 2010 but relapsed again in July 2013 and admitted himself for detox but then discharged against medical advice.
