Failure to report DUIs to board costs counselor Kathleen Davis her couseling license

On December 28, 2012, licensed counselor Kathleen S. Davis entered into an agreement with the Arizona Board of Behavioral Health Examiners to not renew her license when it expires and is prohibited from practicing under her license or providing any type of psychotherapy while it is still valid. The Board’s document states that in May 2006, David disclosed a 1984 DUI conviction on her licensure application and that while the application was pending, she was arrested in December 2006 and charged with extreme DUI after her vehicle collide with oncoming traffic. Her blood alcohol level was .238 one hour after the incident. She failed to report her extreme DUI arrest and subsequent conviction to the Board, as required, until she submitted her March 2009 renewal application.
