Wisconsin Medical Board reprimands psychiatrist Michael Witkovsky

On September 21, 2011, the Wisconsin Medical Examining Board reprimanded psychiatrist Michael T. Witkovsky for unprofessional conduct.

According to the Board’s Final Decision and Order, Witkovsky was employed by Group Health Cooperative of South Central Wisconsin (“GHC”) and Madison Psychiatric Associates (”MPA”), both located in Madison, Wisconsin.

On October 21, 2009, GHC notified Witkovsky by letter that his employment contact had been terminated. The basis for the termination was that he had 345 open medical records which he had not completed in a timely manner.

On April 28, 2010, MPA notified the state Department of Health that Witkovsky ended his practice with them on April 2. Witkovsky failed to maintain adequate records, including not completing initial evaluations, progress notes, or termination summaries.
